  1. Population and dissenters: I have about five & Twenty Families in my Parish in which there are no Dissenters of any sort: neither is there any Licensed or other Meeting House of Dissenters in my Parish.

  2. Public or charity schools: There is no Publick nor Charity School in my Parish.

  3. Alms-house, hospital or charitable endowment: There is not in my Parish any Alms House Hospital or other Charitable Endowment: neither have any Lands or Tenements been left for ye Repair of my church or other Pious Use.

  4. Residence upon cure: I Reside personally upon my Cure and in my Parsonage House.

  6. Divine service at other churches: I do not perform Divine Service at any Church besides my own.

  7. Frequency of divine service: Divine Service is performed in my Church Twice every Lords Day with a sermon in the morning and Generally in Afternoon also. I also perform Divine Service on the Days of Humiliation & Thanksgiving appointed by Autority.

  8. Frequency of communion: The Holy Sacrament of ye Lords Supper is administred in my Church Three Times in ye year at Easter, Whitsuntide, or Trinity Sunday, and at Xmas.

  9. Number of communicants: There are about Forty Communicants in my Parish; of This Number Twenty Generally Receive ye sacrament and I believe as many Received at Easter Last.

  10. Catechising of children: I Did Catechize The Children of my Parish, and also Expounded ye Catechism instead of preaching; but very few Children coming to be catechized, and my Congregation growing Less, on Account of my Reading an Exposition of the Catechism in ye Desk instead of a sermon in ye Pulpit, I have Omitted This good Custom of Late; but intend to resume it again as soon as I can form a Course of Sermons on Catechetical Heads.

  11. Chapels or chapels in ruins: I have no Chappel nor Ruins of a Chappel in My Parish.
